Membership Application Form Adults

There are two ways to apply:

• Download the PDF form (at the bottom of this page), fill in the form on a PC, print, sign, and send us the signed form by traditional paper-mail (our address is at the bottom of this page).
• Or use the on-line form below. You can fill it in on a PC or mobile phone. Printing is not necessary. Much more convenient!

Form for adults

We use an on-line form by Adobe Acrobat. Please click on the picture below to start.

1. Accept the terms and conditions
2. Fill in the mandatory fields
3. Sign by typing your name
4. Follow the instructions to complete signing procedure.
5. You will recieve a email with a link. Click on that link to verify your email address.

The membership adminstration will then return the signed application form to confirm that we received the application.
